Description: This course presents an overview of person-centered planning, a variety of thinking tools that can be used to help structure person-centered planning, four common approaches to person-centered planning including Essential Lifestyle Planning, Personal Futures Planning, Making Action Plans (MAPs), and Planning Alternative Tomorrows with Hope (PATH), and strategies to facilitate successful person-centered planning meetings.
